Admission Information

We understand that admission day can be a difficult time, both for new residents and their families. Our process is designed to make the moving experience as comfortable as possible.

To make the transition easier, please bring the following documentation:

  • Medicare and / or Medicaid card
  • Secondary insurance card
  • Living will and / or power of attorney
  • Emergency contact information
  • Funeral home and burial information

Each patient is an individual and has different needs and wants. Our facility will respect your needs and adapt our care and services to meet your individualized care and personal preferences. It has been our experience that bringing a few personal items can make a new resident more comfortable. We suggest bringing several changes of comfortable clothing, family photographs, books and other personal items to help the resident feel at home.

We accept many forms of payment, including Medicare, Medicaid/Medicaid Pending and private insurance.

It is the admission policy of Autumn Meadows that residents will be admitted, have their rooms assigned, and receive patient services without regard to race, color, national origin, disability or age.
